COVID-19 Appointment Information

 Here are some helpful reminders and new policy and procedures for keeping your appointments and still keeping you safe.


-Please RESCHEDULE  your appointment if you feel AT ALL UNWELL

-Please RESCHEDULE your appointment if you feel at all UNEASY about being in PUBLIC SPACES during this time.

-The MAIN OFFICE DOOR WILL REMAIN LOCKED UNTIL 10 MINUTES BEFORE YOUR APPOINTMENT TIME. Please arrive 10 minutes earlier than your appointment time. Kayla will open it once cleaning is finished and prepared for your safety.

-You may be asked to ALLOW ME to take your temperature BEFORE entering my treatment room.

-Please WASH your hands AS SOON as you enter the office and BEFORE you leave.


-Do NOT bring anyone else inside the office with you.

-You will be required to fill out a COVID-19 form EVERY VISIT. If you REFUSE to fill out paperwork, service will NOT be provided. YOU CAN PRINT THIS OFF UNDER THE HELPFUL FORMS TAB AND BRING IT WITH YOU FOR QUICKER PROCESSING.

 -Regarding Credit Cards, if you feel uneasy about using my card reader and signing the tablet, I will be happy to provide you with an email invoice to pay online. Cash or Check is also accepted.

-Lastly, I ask for your continued patience. As I reschedule please understand I need to make adjustments and will be operating on LIMITED HOURS to ensure physical distancing and clean working space.
